Customer Service: Service Requests - Certificate of Insurance
Certificate and Evidence of Insurance requests can be submitted via FAX (310) 221-0966 or email at certs@nhcinsurance.com. Please click here to download the Certificate Request Form. After completing the form with all of the requested information, please fax to Leana at 310-221-0966 or 310-221-0967.

If you received a letter requesting the certificate, please include that along with your request. This will help ensure that the document(s) meet the requestor's requirements.

Please allow 24 hours* for general certificates and 48 hours for a certificate requiring endorsements and/or waivers.
